If this is for a distance session, I will send you intake forms by e-mail, fax or mail.  Or if feasible, we can do the intake interview on the phone.  Distance sessions will also require that you send me a full-body photograph.  Payment will be handled before the session.  Once these steps have been taken, we can set up an appointment., and I will provide you with details regarding the session at that time.
